Case report of drug-induced liver injury (DILI) developing within four weeks of tirzepatide initiation in a woman in her 20s, presenting as acute hepatocellular injury with extensive workup excluding alternative causes. Tirzepatide is not currently recognized as causing hepatotoxicity. Documents a rare but potentially serious hepatic adverse event requiring clinical awareness—contributing to emerging DILI pharmacovigilance for tirzepatide as a case where routine GI side effects may delay recognition of more serious hepatic injury.
