Case report of a 47-year-old male with obesity and T2DM who developed ecchymotic petechial rash with severe thrombocytopenia within one week of increasing tirzepatide to 12.5 mg, diagnosed as drug-induced immune thrombocytopenia (ITP). No prior ITP history; extensive workup excluded alternative causes. Documents the first reported case of tirzepatide-associated immune thrombocytopenia—a rare but potentially serious immune-mediated hematological adverse event not previously recognized in the tirzepatide pharmacovigilance literature.
