Case report of a 53-year-old man with pre-existing asymptomatic diverticular disease who developed complicated diverticulitis with sigmoid stricture, obstructive diverticulitis, and hepatic abscesses during tirzepatide therapy, following rapid 25 kg weight loss. Multi-organ dysfunction resulted from infectious sequelae of the diverticular complication. Documents tirzepatide-associated complicated diverticulitis progression as a rare but severe GI adverse event—raising the possibility that rapid GI motility changes, altered bowel microbiome from weight loss, or direct drug effects on colonic physiology may precipitate diverticular complications in predisposed patients.
