Case report of euglycemic diabetic ketoacidosis in an 18-year-old woman without diagnosed diabetes who self-administered counterfeit semaglutide obtained without medical supervision for weight loss. Reduced oral intake combined with GLP-1 RA-induced GI side effects triggered starvation ketosis. Illustrates the dual danger of counterfeit semaglutide products and unsupervised GLP-1 RA use for cosmetic weight loss—highlighting regulatory and public health concerns about the black-market semaglutide supply chain and the serious metabolic complications in non-diabetic users.
