Case report of acute pancreatitis developing in a 59-year-old man with T2DM, colon cancer, stage 4 CKD, hypertension, and dyslipidemia after 2.5 years of semaglutide therapy. Contributes to the small but concerning database of semaglutide-associated pancreatitis cases—particularly notable given the patient's multiple comorbidities and prolonged exposure before onset. Reinforces the importance of monitoring amylase/lipase and recognizing pancreatitis symptoms in long-term semaglutide users, especially those with additional pancreatic risk factors.
