Case report of a morbidly obese patient (BMI 39.1) receiving oral semaglutide for preoperative weight optimization before living donor liver transplant, achieving sufficient weight loss to achieve a safe graft-to-recipient weight ratio and complete the transplant safely. Documents successful use of semaglutide as a bridge to hepatic transplantation. Provides the first case-level evidence for oral semaglutide as a preoperative weight loss intervention in liver transplant candidates—establishing a clinical rationale for GLP-1 RA use in the liver transplant program where weight thresholds determine surgical eligibility.
