Complex clinical case report of a young adult with psychiatric comorbidities and Munchausen syndrome-like behavior presenting with recurrent emergency episodes, discovered to be covertly self-administering multiple concealed medications including semaglutide to induce hypoglycemia, hypotension, and clinical deterioration. Documents deliberate semaglutide misuse as a mechanism for factitious disorder-related hypoglycemia. Provides clinical awareness for emergency and psychiatric physicians that semaglutide can be deliberately misused to induce hypoglycemic episodes—expanding the differential diagnosis for unexplained recurrent hypoglycemia in patients with psychiatric histories.
