This study evaluated the immune effects of mediastinal radiation therapy in lung cancer patients enrolled in a thymosin alpha-1 trial. After radiation, patients showed significant drops in T-cell numbers (all subsets), altered CD4/CD8 ratios, and impaired T-cell function in mixed lymphocyte cultures. Short-term in vitro incubation with thymosin alpha-1 enhanced immune responses in 12 of 27 patients with post-radiation abnormalities. In placebo-treated patients, T-cell depression persisted for 3-4+ months, and progressive T-cell subset declines always preceded clinical relapse.
